A free, community health worker–led program aimed at reducing infant mortality and improving maternal-infant health. Covers services from pregnancy through the first birthday Offers care coordination, health education, and help with:

  • Baby items (cribs, car seats)

  • Food assistance

  • Transportation

  • Employment or housing support

  • Medication access, immunizations, substance use support
