By completing the screenings described on this page, your child will be enrolled in the OberlinKids’ program, On Track for Kindergarten, and be eligible for several programs and resources available only to OberlinKids members.

Why Should All Parents & Caregivers Complete the ASQ Every Year?

Even if you have no concerns about your child's development, completing the Ages & Stages Questionnaire (ASQ) annually is one of the best ways to support your child's growth. Here’s why:

Confidential Information

Rest assured that all information is kept confidential within the collaborative. Jenn Keathley, the Director of OberlinKids, is the only person who sees all of the information regarding your family and child entered on the questionnaires.

Annual Screening Recommended

Caregivers are asked to complete a new screening at least once per year. A good way to remember is to do a new one during the child's birth month each year.

What’s Involved?

Whether this is the first time completing your child’s annual screening or it is simply time to update your family’s On Track for Kindergarten info by completing your next screening, it is something you can do online from the comfort of your own home in less than 30 minutes!

  • 1. Access the screening forms.

    Click below to access the relevant Ages & Stages Questionnaires® for your child.

    After providing your child’s birth date, you will be presented with the names of and links to the recommended ASQ-3™ and ASQ:SE-2™ forms that are recommended for your child, based on their age.

  • 2. Complete the screening forms.

    The questionnaires are play-based and you may be able to provide answers to some of the questions without involving your child because of the time you have spent playing with them.

    If you don’t know the answer to a question, you may need a prop – like a ball, blocks, or a toy (depending on your child’s age) – to see your child in action.

    📄 Paper Forms. You may print the forms to preview the questions or write down the answers while playing with your child. Then, you can return to the ASQ website to submit your answers online when you are ready. Need assistance with this step? Click here.

    💻 Online Forms. Click the ‘Enter Your Results’ button to submit your responses online. The forms will be presented to you in this way:

    → ASQ-3™ form first, then submit your answers.

    → Click the link to the ASQ:SE-2™ form, then submit your answers.

  • 3. Get the results.

    After submitting the forms online, you will be presented with a link to print additional information: a PDF containing notes about your child’s social emotional development and activities to help them develop their social emotional skills.

    Jenn Keathley will send you an email with copies of the results and recommendations, including age-appropriate activities to continue to learn through play at home and hone skills.

    If there are referrals or supports needed they will be shared in the email and individualized support services are offered to each family.

  • 4. Congratulations, your child is now a member of OberlinKids!

    This means they are enrolled in our program, On Track for Kindergarten, and eligible to participate in our featured programs.

Sharing Screening Results with Families and Early Learning Centers


At OberlinKids, we believe in the power of collaboration to support each child’s growth and development. In alignment with Ohio’s Step Up To Quality Standards, all High-Quality, Licensed Early Learning Centers are required to complete an approved developmental screening for every child within 30 days of enrollment and at least once per year thereafter.

Jenn Keathley, OberlinKids Director, works closely with each of our partner early childhood education centers to share the results of the Ages & Stages Questionnaires® (ASQ®) for every enrolled child. Through a secure and efficient system, screening results and age-appropriate activities are shared directly with center staff and families to help them support children’s development at school and home.

For children not enrolled in a partner learning center, Jenn communicates results directly with the child’s parent or caregiver. Families are encouraged to share these results with anyone involved in the child’s care—such as other family members, pediatricians, or service providers—so that everyone is informed and aligned in supporting the child’s healthy development. Sharing screening outcomes helps ensure every child receives the right support at the right time. Statistically, children who start school behind their peers often struggle to catch up. But when families, educators, and caregivers work together, we can help every child stay on track and be ready for kindergarten—and for a lifetime of learning.
