What is Family Connects?
Family Connects is an evidence-based, universally offered home visiting program designed to support all families with newborns. The model offers a nurse visit at 3 weeks postpartum at no cost to the family to ensure families have the support they need during the critical early months of a child's life. By addressing physical health, emotional well-being, and social determinants of health, Family Connects empowers families and strengthens communities.

Maternal Health: Reducing Maternal Mortality & Morbidity
Maternal mortality and morbidity remain pressing public health challenges in the United States. Among high-income countries, the United States has one of the highest maternal mortality rates with preventable complications continuing to put lives at risk.
Family Connects integrates universal newborn nurse home visits into a broader system of care that addresses the critical needs of families during the postpartum period. By identifying risks early, facilitating access to care, and providing education and support, the program is working to save lives and promote long-term health for babies and their families.
